Calvary Community Care (C3)’s data protection policy

This Privacy Notice (“Notice”) sets out the basis which Calvary Community Care (“C3, we,us” or “our”) may collect, use, disclose or otherwise process personal data of our prospective service users, service users, service users’ authorised representative, donors, volunteers in accordance with the Personal Data Protection Act (“PDPA”). Personal information is given in good faith by stakeholders and will only be used to establish and maintain their relationship with C3.

C3 collects, use and discloses personal data for following purposes:

Clients

  • providing the services requested by you;
  • monitoring, evaluating and/or auditing of services provided. This may include an assessment of the quality of the services provided and the effects of the services provided (both in the short and long term, and after you have stopped using the services);

Donors

  • processing your donations and your tax-deduction claims


Volunteers

  • processing and evaluating your suitability to volunteer; and
  • monitoring, evaluating and/or auditing of services you participated in.

Common purposes across different stakeholders:

  • providing you with information on our upcoming events or activities, where you have specifically requested to receive such information;
  • verifying your identity;
  • responding to, handling, and processing queries, requests, applications, complaints and feedback from you;
  • complying with any applicable laws, regulations, codes of practice, guidelines, or rules, or to assist in law enforcement and investigations conducted by any governmental and/or regulatory authority;
  • any other purposes for which you have provided the information;
  • transmitting to any unaffiliated third parties including our third party service providers and agents, and relevant governmental and/or regulatory authorities, whether in Singapore or abroad, for the aforementioned purposes; and
  • any other incidental purposes related to or in connection with the above.

Retention of your information

C3 will cease to retain your personal data, or remove the means by which the data can be associated with you, as soon as it is reasonable to assume that such retention no longer serves the purpose for which the personal data was collected, and is no longer necessary for legal or business purposes.

Access, Withdrawal and Correction of your information

Access Form

Under the Personal Data Protection Act 2012 (“PDPA”), you are entitled to request for your personal data that we have, and request to know how your personal data has been used or disclosed over the past.

Access to your information is subjected to an administration fee of $20. PAYNOW to UEN T10SS0047C and indicate “PDPA Access” in payment reference. Do note that we will take up to 30 days to process your request and will inform you via email if more time is needed.

Correction Form

Under the Personal Data Protection Act 2012 (“PDPA”), you are entitled to correct personal data that we have collected from you.

We may refute your access in cases if and when such related personal data is under legal proceeding, no proper identification or request is carried out with suspected fraudulent intent. Some of these circumstances include (i) the exceptions under the Fifth Schedule of the PDPA; and (ii) the prohibitions under section 21 of the PDPA.

Do take note that withdrawal of personal data as client or volunteer would result in C3 not able to continue fulfilling our services to the client or engaging you as our volunteer.
